1. These Terms and Conditions apply to the Latrobe Health Services “InstantScripts Pilot” promotion (“Promotion”). You must comply with these Terms and Conditions and meet the criteria in clause 4 to be eligible for the Promotion.
2. Latrobe Health Services is offering access to InstantScript (InstantScripts Pty Ltd (ACN 629 120 234) General Practitioner Telehealth services (“Doctor Consultation”) if you meet the eligibility criteria in clause 4 (an "Eligible Person").
3. This Promotion is offered by Latrobe Health Services Limited ABN 94 137 187 010 of 32 Darlimurla Avenue, Newborough VIC 3825. (“Promoter” or "Latrobe Health Services”).
4. This Promotion is only open to people who:
- reside in Australia;
- have been identified as a trial participant membership by Latrobe Health Services;
- have received a referral code directly from Latrobe Health Services offering access to InstantScripts
- are not be a director, officer, management or employee of the Promoter or its related entities and must not have any immediate family members who are employees of the Promoter or its related entities. Immediate family means spouse, de facto spouse, child or stepchild (whether natural or by adoption), parent or stepparent, brother, sister, step brother or step sister.
5.Upon signing up for the InstantScript service, Eligible Persons will need to agree to the InstantScript Terms and Conditions.
6. Latrobe Health Services, as part of this Promotion, agrees to provide Eligible Persons with a fully subsided General Practitioner Telehealth Service (“Doctor Consultation”) provisioned by InstantScripts. Latrobe Health Services does not offer any promotions on non ‘Doctor Consultation’ services offered by InstantScripts
7. This promotion is limited to Eligible Persons listed on an identified trial participant membership and cannot be used by non-Eligible Persons. Latrobe Health Services may refuse subsidisation for non-Eligible Persons.
8. Nothing in these terms and conditions limits, excludes, or modifies your rights under Australian Consumer Law or similar laws, including implied warranties and protections against unfair practices. These rights are referred to as "Non-Excludable Obligations."
Except for any liability that cannot be excluded by law, including the Non-Excludable Obligations, Latrobe Health Services is not liable for any indirect or consequential loss or damage you may suffer in connection with this service or its use, including loss of opportunity, loss of profit, personal injury, or property damage.
