Mail your completed claim form and original provider invoices for a fast mail service. If you are claiming for medical services provided while you were in hospital, attach the Medicare Statement of Benefits.

Medicare Two-way Claim Form

Medical claims – forms to be completed.

All Medical claims will require the following forms to be completed.

• Where the patient lodges the claim at a Medicare office, they need to complete a Medicare claim form and a Medicare Two Way claim form, attach the original accounts and receipts if paid.
• Where the patient lodges the claim with Latrobe Health Services the member completes the private health fund’s claim form and a Medicare claim form, attaches the original accounts and receipts if paid.
• Paid accounts the Patient takes account direct to Medicare and then completes a LHS claim form and attach the Medicare Statement of Benefits.

Please note that if your account is lodged with Medicare in the first instance no additional benefits can be paid without a LHS claim form being completed. Therefore we encourage you to submit your claims to LHS in the first instance to ensure maximum benefit is paid.
